The website is www.empathhealth.org and the Advance Care Planning tab includes downloadable living wills in English and Spanish, Interactive Living Wills, Dementia Living Wills, resources, etc.
To call for an Advance care planning packet please dial 727-586-4432. The packet includes 2 living wills with the 24 page The Greatest Plan booklet and a booklet for the HCS called “what do I do now”?
